Notes
1. The Physician or Physician Assistant will exercise discretion concerning
the advisability of any particular care or testing. The Provider will
also advise the Patient when any care or testing is not covered under
the PCA contract.
2. Non-routine testing is not included. Examples of these kinds of test
include specialized genetic testing (such as Tay-Sachs hemochromatosis
etc.) antibody titers and special immunology testing. Non-routine tests
will be billed directly to the Patient.
3. Other travel related vaccines (such as Typhoid and Hepatitis A and
B) are available for an additional fee.
OUR
SERVICES
Under the Personal Care Associates contract, PCA will
provide the Patient with routine care and testing as determined by the
Patient’s Provider. This comprehensive program includes
the following (when appropriate):
*One physical examination annually
*Routine office visits
*Urgent visits, including routine cultures and necessary labs
*Appropriate telephone and e-mail communication
*Relevant medical research and educational materials
*Laboratory testing:
Blood count
Chemistry panel: for diabetes,
liver, kidney and thyroid function
Cholesterol
Urinalysis
Prostate cancer screening (PSA)
Hemoccult cards
HIV screening [if requested]
Blood clotting times
Cardiac risk screening (CRP, Homocysteine)
*Routine Cultures:
Throat
Urine
*Office Procedures:
EKG
Pulmonary
function
Tuberculosis skin testing (PPD)
Body Fat Analysis
Pap Smear [when indicated]
*Immunizations
Tetanus and Diphtheria
Annual Influenza
Pneumonia
