FEE SCHEDULE

The base charge for the provision of Covered Medical Services by Personal Care Associates will range from $1800 - $2600 per year. The fee will be determined by your physician after assessment of your past, present and ongoing healthcare needs. The fee is targeted to reflect a regular "fee for service" experience that is tailored to meet your individual needs. The annual fee may be paid in one initial payment or you may establish a payment arrangement of three consecutive monthly payments.

After each visit you will be mailed a claim form. These forms may be submitted to your insurance company for reimbursement. Contact our administrator, Mary Doherty, or our billing manager, Wendell Davis, for assistance.
